How to sign your Notarize or Commission a Sworn Statement for a Family Gift of a Used Vehicle?



This document is required where one person who owns a vehicle registered in Ontario; wishes to transfer it to a qualifying family member on a tax exempt basis. When visiting Service Ontario to complete the transfer of the vehicle, this document should be presented along with the signed ownership and any other documents required by Service Ontario. After you have filled out your document, it's time to sign it BUT REMEMBER you MUST sign your completed document in the presence of a Notary Public or Commissioner for Taking Affidavits. HOW DOES IT WORK?

  1. BOOK YOUR APPOINTMENT: Click here (or use the "Book Now" button below) to Schedule your online commissioning appointment via Zoom;

  2. BEFORE OUR APPOINTMENT:  Please access your confirmation e-mail and click the "UPLOAD DOCUMENT" button to submit the document(s) you need notarized (commissioned), your ID and provide some additional details for our appointment;

  3. AT OUR APPOINTMENT TIME:  Please access your confirmation e-mail and click the "Join Video Conference" button to join me in a short Zoom meeting;

  4. DURING OUR APPOINTMENT:  Please have your original government issued photo ID with you during our appointment.  Once your ID has been verified, I will provide a link to view and sign your document electronically and will administer an oath, affirmation or declaration; 

  5. AFTER OUR APPOINTMENT:  You will receive your electronically signed and commissioned document within 5 minutes of our appointment ending.  If you would like an original with embossed seal, I it will be mailed or couriered to you immediately after our appointment.
